Jonathan found his passion for helping others alleviate pain and accomplish their functional goals to return to the active lifestyle that individuals desire while finishing up schooling at Stony Brook University. While working at an outpatient physical therapy clinic, specialized in treating neurological conditions, Jonathan finished up his Doctorate from Husson University in Bangor Maine.
Jonathan assisted in the opening and development of the Fleetwood Professional Physical Therapy clinic in 2018. The goal was to create a warm and welcoming environment implementing evidence-based treatment using manual therapy, exercise and active treatment, and patient education. The office continues to successfully treat and allow return to optimal health and function for countless individuals each year.
Dr. Basile is a member of the American Physical Therapy Association (APTA) and actively participates in education as a clinical instructor. Additionally, Jonathan’s research was published in the Journal of Orthopaedic and Sports Physical Therapy (JOSPT).
Jonathan continues to treat and care for individuals with a wide array of needs using a personalized and individualized approach.
Kevin Baltazar, PT, SFMA

Kevin has 6+ years in Outpatient Orthopedic setting. Kevin is currently SFMA 1 and 2 certified and BFRT (Blood Flow Restriction
Training) certified. Additionally, he enjoys finding and treating the source of pain/dysfunction and getting the patient to actively
participate in their road to optimal pain-free performance. He can talk for hours about coffee, good food, philosophy, and his wife and 2 children.
Anne Patti, PT, DPT

Anne received her bachelors degree in Exercise Science from University at Buffalo in 2017 and went on to receive a doctorate degree
in Physical Therapy from Rutgers University North Program 2020. During her clinical rotations, she had the opportunity to work in
general orthopedics, oncology, pelvic health, and pediatrics. Throughout her physical therapy education, Anne trained and taught
classical Pilates group and private classes through a studio in NJ. Anne is now a resident in ProPT-NYIT Orthopedic Residency
